Little Red Door Cancer Agency (LRD) seeks a full time Client Navigator to serve as case manager to cancer patients seeking LRD assistance in successfully navigating their healthcare and maximizing their overall wellbeing during care and into survivorship.

Organization Overview
Little Red Door seeks to make the most of life and the least of cancer for central Indiana residents by working to reduce the physical, emotional, and financial burdens of cancer for medically underserved residents of central Indiana by providing free client services, cancer screenings, and education.

Position Summary
The Client Navigator will be directly responsible for completing needs assessments, partnering with clients to successfully complete their service plans, and navigating clients through the healthcare and social service systems to access cancer screenings, cancer treatment, support services, affordable health coverage, and other needed resources.  Navigator is expected to build strong, supportive, long-term relationships with assigned clients. This position serves as an active member of Little Red Door's Programs team and will report to the Director of Client Services.

Essential Duties and Responsibilities include the following (other duties may be assigned to meet business needs):

• Educate clients about cancer prevention, screenings, treatment, affordable health coverage, and the importance of follow-up services.
• Perform needs assessments and service planning.
• Connect clients to LRD provided services such as nutrition assistance, transportation, Door to Wellness programming, and personal care supplies.
• Connect client to community resources to fulfill needs and overcome barriers.
• Act as a liaison between clients and community providers and resources to fulfill client needs.
• Provide practical and emotional support to clients in successfully navigating the healthcare system and completing cancer treatment.
• Collect and enter client information in a database including contact, demographic, and health information and complete program enrollment paperwork.
• Attend and engage clients in Tuesday and Thursday Door to Wellness programming.
• Attend and engage clients in Monday evening Door to Wellness programming once per month.
• Handle confidential and non-routine information and explain procedures when necessary.
• Organize and prioritize client information to ensure clients' needs are met in a timely manner.  
• Routinely follow-up with clients that have received services to provide additional assistance. 
• Perform fittings for wigs and breast prostheses with appropriate training.
• Create or enhance collaborations with providers to foster client advocacy and visibility of program services.
• Work independently and within a team on special and nonrecurring and ongoing projects, special assignments and tasks under the general supervision of the Director of Client Services.
• Other tasks and duties as assigned by the Director of Client Services. 
• Specialize in assigned cancer types by staying up to date on the latest research, understanding cancer treatment and side effects, and identifying cancer-specific resources. 

Preferred Duties and Responsibilities
• Serve as the primary point of contact for all Spanish-speaking agency clients.
• Attend and participate in outreach and educational activities targeted toward Spanish-speaking and minority populations to promote Little Red Door's services.

Knowledge, Skills, & Abilities
• Fluent in Spanish and English, both written and oral highly preferred, but not mandatory.
• High level of comfort with needs assessments and service planning.
• High level of comfort with and motivation for creative problem-solving and solution planning.
• High level of familiarity with Indianapolis resources.
• High level verbal, written and interpersonal communication skills and phone etiquette to handle sensitive and confidential situations.
• Strong organizational skills and detail oriented, continual attention to detail in collecting and entering client information and prioritizing client needs through phone and face-to-face interaction.
• High degree of professionalism and positive attitude, demonstrated poise, tact, diplomacy and compassion for clients. 
• Exceptional degree of reliability and punctuality.
• Ability to understand and carry out written and oral instructions effectively.
• Self-directed and assertive with skilled decision making.
• Ability to complete tasks with minimal assistance and supervision; must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks. 
• Must be able to interact and communicate with individuals at all levels of the organization.
• Must have knowledge of a variety of computer software applications in word processing, spreadsheets, and database software (Outlook, MSWord, Excel).

Education / Experience
• BSW or similar social services degree. Case management experience may substitute for education on a case-by-case basis.
• Case management or healthcare experience preferred.
