We are looking for an experienced case manager for our family homeless shelter program!  Family Promise of Greater Indianapolis (FPGI) is a partnership of congregations and community organizations responding to the crisis of children and their families who are homeless. We work to eliminate homelessness in Greater Indianapolis through our Interfaith Hospitality Network (IHN) shelter and our AfterCare program. The offices are located at the IHN Day Center at 1850 N. Arsenal Avenue, Indianapolis, IN 46218. The goal of the Shelter Case Manager position is to help approximately 65 families each year move from homelessness to permanent housing and greater financial stability.  The position reports to the Executive Director.

Interfaith Hospitality Network (IHN) is the core program of Family Promise of Greater Indianapolis.  Its purpose is to provide temporary emergency shelter to children and their caregivers who are experiencing homelessness.  It achieves this by coordinating a network of 35 religious congregations in and around Indianapolis to host up to eight families at night, while providing case management and other resources at the Day Center (1850 N. Arsenal Ave., Indianapolis).


Case Management
• Conduct phone screening and interviews to keep shelter as close to full capacity as possible while ensuring mutual appropriateness of fit between families and program.
• Orient guests (i.e., families) to program policies and expectations upon arrival.
• Meet with families multiple times weekly to assess needs, set goals, create plans of action, support with resources and referrals, and evaluate progress.
• With a mindset of empowerment over enabling, constantly assist (or coordinate others to assist) families with work towards goals, including providing coaching for job interviews, financial literacy training, parenting skill development, educational system navigation, and more.
• Attend and present case planning notes and obstacles at weekly case conferences.
• Complete and submit prevention, rapid re-housing, and permanent supportive housing applications as needed through the Indianapolis Continuum of Care.
• Record thorough case notes and tracks outcomes in HMIS.
• Maintain relationships with other agencies and nonprofit organizations.

Other Responsibilities
• Provide phone and door reception service when scheduled at the Day Center, typically but not limited to one 3-hour shift per day.
• Facilitate weekly house meeting of all guests at the Day Center.
• According to the on-call schedule, covers with on-call phone every third week.

Other reasonable duties may be assigned as needed, conditional on priority and availability.

•       Ability to focus, prioritize, and maintain calm and flexible in fast-paced and dynamic work environment.
•       Strong communication skills in a team work environment.
•       Ability to pitch in with all staff and guests and take responsibility for our shared space.
•       Respect for confidentiality of personal and sensitive information.
•       Knowledge of local community resources.
•       Set a fluctuating schedule of 40 hours/week that enables highest probability of meeting client needs (could include weekend and evening hours).
•       As needed (generally once per week) arrive as early as 6am to open Day Center.
•       Bachelor of Social Work degree (5 years equivalent experience accepted); Masters a plus.
•       Two years case management experience in a social services setting.
•       Proficiency in Microsoft Word, Excel, and Outlook.
•       Experience with HMIS database for tracking a plus.
•       A valid driver's license and reliable vehicle.
•       Able to walk up and down stairs; able to lift up to 30 pounds.
