This Higher Ground Case Manager provides a continuum of care for long-term guests in the Higher Ground Addictions Recovery Program at Wheeler Mission's Center for Women & Children by assisting participants through case management, education, and emotional/spiritual counseling during the addictions recovery process.

DUTIES
The Higher Ground Case Manager's (HGCM) duties include tasks within the following areas:
a. Case Work: Provides case management services for a diverse population of women that may include, but is not limited to, assessment of client's strengths, resources and needs, crisis intervention, information and referral, all directed toward achieving total stability for each client.
b. Coordination of Services: Will work corroboratively with other staff members and the client to help the client achieve goals. The HGCM will work with the Case Managers from other phases of the Higher Ground program to coordinate care for the program participants and allow for ease of transition.  The HGCM will also work with the Education Coordinator, Program Director, appropriate outside providers, CWC Director and other case managers to coordinate care for those in the Higher Ground program.
c. Planning: Develop a care plan with written goals and methods to help the client achieve the Benchmarks determined for the Higher Ground program. The HGCM will monitor the effectiveness, appropriateness and efficiency of the plan, modifying as needed.
d. Record Keeping: Maintains appropriate documentation of client sessions for identifying information, agency referral and follow-up. The Case Manager will retain this information on a database that can easily be shared when needed.
e. Legal: Takes appropriate action according to the established protocol of the organization, maintaining client confidentiality in all areas following biblical definitions of ethical conduct and behavior.
f. Administrative:  Attends staff meetings and completes other duties as assigned by their supervisor. 

Qualified candidates for the Higher Ground Case Manager (HGCM) position will possess the following qualities or experience:
a. The HGCM must possess a sincere love for the Lord and a desire to see individuals come to a personal relationship with Christ and grow in Him.
b. The HGCM must be in agreement with the philosophy and policies of the ministry and is able to view this as a team ministry with a servant's heart.
c. The HGCM must possess a minimum of a BA/BS in a related field of study or equivalent experience.
d. The HGCM must be flexible and able to work with homeless women from various backgrounds and at different skill levels and have experience working with addictions.
e. The HGCM must have experience working with women and children who may have experience substance abuse, mental illness, trauma, domestic violence, or homelessness.
