Family Promise of Greater Indianapolis (FPGI) is a partnership of congregations and community organizations responding to the crisis of children and their families who are homeless. We work to eliminate homelessness in Greater Indianapolis through our Interfaith Hospitality Network (IHN) shelter and our AfterCare program. The offices are located at the IHN Day Center at 1850 N. Arsenal Avenue, Indianapolis, IN 46218. The goal of the Case Manager (Interfaith Hospitality Network) position is to help approximately 70 families each year move from homelessness to permanent housing and greater financial stability.
Interfaith Hospitality Network (IHN) is the flagship program of Family Promise of Greater Indianapolis.  Its purpose is to provide temporary emergency shelter to children and their caregivers who are experiencing homelessness.  It achieves this by coordinating a network of 38 religious congregations in and around Indianapolis to host up to eight families at night, while providing case management and other resources at the Day Center.
Intake and Case Management
• Conduct phone screening and intake appointments to keep shelter as close to full capacity as possible while ensuring mutual appropriateness of fit between families and program.
• Orient guests (i.e., families) to program policies and expectations upon arrival, with a willingness to pick families up if there is no other transportation option.
• Meet with up to eight families once weekly to assess needs, set goals, create plans of action, support with resources and referrals, and evaluate progress.
• With a mindset of empowerment over enabling, daily assist families with work towards goals.
• Attend and report at weekly case conferences with Case Manager (AfterCare) and Executive Director.
• Complete and submit prevention, rapid re-housing, and permanent supportive housing applications as needed through the Continuum of Care.
• Record intake and exit data in HMIS (database system).
Volunteer Coordination
•         Send Family Information Sheets weekly to upcoming congregational coordinators.
•         Email and call coordinators as needed to communicate changes to Family Information Sheet.

• Provide phone and door reception service when scheduled at the Day Center, typically but not limited to one 3-hour shift per day.
• Conduct at least one opening and closing of the Day Center each week as early as 6:00am (preferably Mondays), leaving at 2:30pm.
• Facilitate weekly house meeting of all guests at the Day Center.
• Visits congregations approximately one evening every two weeks, coordinating with Case Manager (AfterCare) and Executive Director.
• According to the on-call schedule, covers with on-call phone every third week.
• Other reasonable duties as assigned.
•         Empathy for our guests experiencing homelessness.
•         Ability to work effectively in a diverse, dynamic, and team environment.
•         Respect for confidentiality of personal and sensitive information.
•         Knowledge of local community resources a plus.
•         Bachelor of Social Work degree (degree of comparable field or equivalent experience accepted)
•         One year case management experience in a social services setting.
•         Proficiency in Microsoft Word, Outlook, web browser software, and Google Maps.
•         Experience with HMIS database for tracking a plus.
•         A valid driver's license and use of a reliable vehicle for traveling to/from visits to congregations and workgroups (mileage reimbursement provided).
•         Willingness to drive (or learn to drive) a 15-passenger van (no CDL is required).
•         Able to walk up and down one flight of stairs approximately five times per day.
•         Able to lift up to 30 pounds.
