Essential Responsibilities
• Maintain an active caseload of HIV+ clients receiving long-term housing assistance
• Collaborate with Care Coordinators about client care plans
• Interview and assess clients' barriers to reaching independent housing sustainability, including discussing employment options and resources with client
• Assist clients in locating affordable housing opportunities
• Assist clients in developing budgets appropriate for the household needs/income
• Formulate care plans and generate referrals to address specific client needs 
• Schedule and Instruct "Building-A-Future" life skills classes as needed
• Perform housing inspections as needed
• Complete check requests for approved expenses
• Attend monthly housing assistance meetings
• Participate in Continuum of Care meetings and processes, including Coordinated Entry
 
Administrative Duties
• Complete necessary paperwork and data entry in a timely fashion
• Collect necessary documentation for program eligibility (ID, POS, Lease, Utilities, Income)
• Maintain client records and update client records according to program requirements
• Prepare regular reports regarding client progress for committee review
• Perform effectively within the organization and interpersonally with co-workers
• Attend staff, department, team, and other requested meetings
• Other duties, as needed, or directed by Housing Program Manager
 
Skills
• Strong verbal and written communication skills
• Above average skills using Microsoft Office Suite (Outlook, Excel, Word)
• Knowledge of the trends in the nonprofit housing sector
• Ability to establish and maintain relationships with people from diverse backgrounds
 
Education
• Bachelor's degree in a social services related field preferred
 
Experience and Knowledge 
• Housing services as related to the HIV population
• Case management
• History of working with diverse populations (LGBT, Communities of color, substance users, commercial sex workers, etc.)
• Group facilitation
 
Physical Requirement
• Ability to sit for extended periods of time
• Ability to work specific evenings and weekends as needed
• Provide own transportation as needed
• Ability to lift a maximum of 50 pounds
