Position Goal to Support Mission:
Reduce the likelihood of recidivism and crime by providing a team of expert professional support to address re-entry challenges in the areas of housing, job readiness, employment and education.

Position Summary:
The R3 Case Manager will have significant responsibility for the implementation of the R3 Re-entry Program at CAFÉ and will work closely with clients from the on-site parole office, the Boys and Girls Club Re-Engagement Center and individuals who walk in seeking services. The R3 Case Manager will leverage a collaborative relationship that refers individuals working with partnering agencies to enroll them into the R3 program.

The R3 Case Manager will provide wrap-around services to support clients who experience barriers because of a criminal history. This includes administering a complete comprehensive assessment that accesses 12 different life areas. Through the assessment the R3 Case Manager will ensures that they are aware of and understand the participants unique barriers and challenges. The R3 Case Manager will be responsible for helping participants to create short and long-term goals that address barriers and challenges.

Based on those findings the R3 Case Manager will create and provide a comprehensive re-entry plan that is individualized and focuses on areas of job readiness, housing assistance, financial coaching and intensive follow-up case management to ensure program participants receive assistances for other areas of need that may be discovered throughout their involvement in R3.
The R3 Case Manager will report directly to the Chief Executive Officer and work closely with other CAFÉ staff such as the Career Coach, Financial Coach, Family Support Coach, the Behavior Health Specialist and Employment Specialist.

Essential Functions:
Program Implementation 
• Provide wrap-around services to support clients who experience barriers because of a criminal history
• Administer a complete comprehensive assessment that assesses 12 different life areas
• Help participants create short and long-term goals that address barriers and challenges
• Develop a follow-up plan that tracks the progress of participants
• Provides direct connections and support in areas of housing, job placement, financial coaching, education and one-on one counseling services

Data Management and Reporting
• Understand data management protocols and procedures to ensure timely data entry and reporting
• Review and update client data records 
• Generate individual and aggregated reports to keep clients and program management informed about current trends and outcomes

• Enroll 60 clients who have been released in the last 180 days into R3 program
• 100% complete re-entry plan
• 42 employed
• 26 employment retention
• 48 increase net income, net worth or credit score
Program Integration
• Work as a member of the cross-functional team providing services to families enrolled in the CWF initiative, sharing information, participating in team meetings, and helping to track and report progress and outcomes related to the CWF Initiative.

Program Development
• Review best practices and stay up to date on the Re-entry industry practices 
• Work in partnership with Program Director and other team members to develop researched based programs and services
• Work with others to continually identify areas of improvement or opportunity

• Degree preferred but not required. Relevant work experience working with re-entry and vulnerable populations with proven ability to successfully use coaching skills.
• General knowledge and understanding of the needs of the re-entry population, as well as knowledge of available community resources.
• Experience teaching or facilitating group workshops is a plus.
• Excellent social/interpersonal skills and writing ability commensurate with the communication and reporting requirements of the position.
• Strong computer skills including experience with MS Office applications, using the Internet and database management.
• Well-organized and self-directed, with ability to manage multiple tasks and priorities with great attention to detail.
