Noble is adding a full-time Assessment and Referral Manager to our talented team of staff who will be responsible for coordinating the referral and intake process for individuals seeking services from Noble. The primary function of this position is to assist in the planning and coordination of services, supports and resources for individuals to ensure the implementation and achievement of quality services and outcomes.

Specific responsibilities include:
• Provide intake, initial assessment and follow-up services for individuals referred to Noble.
• Complete Person-Centered Planning during the intake meeting and provide solution-based exploration for the individual and their family to determine, based on their needs, interest and desires, what services Noble offers which would best meet those needs.
• Track efficiency of intake process, particularly the number of days from initial contact to the intake appointment and internal staffing meeting, and number of days to the actual start of services.
• Maintain in-depth knowledge of current services and keep abreast of upcoming changes to service delivery models.
• Collect thorough and accurate information on individuals seeking services, updating information as needed between initial intake and actual start of services.
• Follow-up with individuals seeking services, family members, providers and case managers if required forms and documentation are not received in a timely manner, or when funding for services has not been approved.
• Provide resources and referral information to potential clients, current clients, families, case managers and staff as needed.
• Represent Noble at community fairs and booths as requested.
• Support families in applying for alternate funding services, i.e. Vocational Rehabilitation, Medicaid Waiver, Choice, Caregiver Support, etc.

Position requires a Bachelor's degree in psychology, social work, education, rehabilitation or related human/social service field; Master's preferred. Minimum of three years of experience working with individuals with disabilities, with emphasis in community services and/or assessment. Ideal candidates will have strong written and verbal communication skills, proficiency with technology, experience with using electronic client databases, and some flexibility in schedule to be able to work occasional evening hours. Must provide own transportation, have valid driver's license and meet driving insurability and background check requirements.
