Provide ongoing support to staff and Individuals with physical and mental disabilities in a group home/Supportive Living/Independent Supportive Living or family home setting. Conducts functional behavior assessments (FBA); designs and implements behavior support programs (BSP). A. Conducts functional behavior assessments (FBA) ; designs and implements behavior support programs (BSP), collects and analyzes monthly data; completes progress notes and other documentation as applicable.
B. Provides individual and group counseling as needed. 
C. Maintains communication with individual's legal guardian/advocate/family regarding social emotional issues, behavioral plans, medication, and progress, etc.
D. Provides consultation and instruction, including in-service education to NH associates and staff from external agencies regarding treatment approaches and techniques.
E. Precepts new associates on the behavior issues and needs of assigned site(s).
F. Participates in the development of individual support plans and attends quarterly and interdisciplinary meetings as appropriate.
G. Responsible for clinical documentation and reports, including psychiatric progress notes, chronos/billing, and monthly summaries.
H. Presents behavior support plans to the Human Rights committee as needed/required.
I. Coordinates, attends, and presents progress report at psychiatric rounds with the consultant psychiatrist.
J. Consults with Level 1 behavior provider on FBA's & BSP's per Medicaid Waiver Standards.
K. Complies with continuing education requirements as prescribed by Medicaid Waiver Program &/or State licensure.
L. Transports individuals to appointments, etc., as needed.
M. Performs other duties as may be assigned.

Minimum requirements are needed in order to perform the essential functions of this position.  Work environment and physical requirements are found on the attached worksheet.
Education:  Master's degree in Clinical Psychology, Counseling Psychology, School Psychology or another applied health service area of psychology, Special Education, Social Work, or Counseling.
Experience:  One year of experience in counseling and/or behavior management working with persons with developmental disabilities is preferred. 
Licensure:  LCSW or LMHC preferred. Must maintain a valid Indiana Operator Drivers License and driving record in accordance with policy guidelines.
Other:  Must be willing to use personal vehicle.  The ability/willingness to work flexible hours is required.  Good verbal and written communication skills are required.
