PACE, Inc. provides services to individuals engaged in the justice system to assist them in transitioning back to their community. 

PACE, Inc.  has been providing services to the ex-offender community since 1960.  The mission of PACE is to provide a variety of services to help persons previously incarcerated to lead productive and responsible lives in their community.  Our vision is to ensure that people returning to the community from incarceration have the tools and resources to successfully reenter; to promote public safety through effective reentry; and to enhance community stabilization through reduced crime and increased productiveness.

POSITION DESCRIPTION:
This is a salaried position in which a person would work at a minimum of 40 hours per week.  This position is contingent upon renewal of grant funds.  The position of case manager is intended to link clients to support services provided by PACE as well as public agencies within the community.  The Case Manager will assess service needs and determine eligibility for referrals for public services.

POSITION REQUIREMENTS: 
• Must have a minimum of a bachelor's degree in criminal justice, social work, or related area of study.  
• Must have experience and/or a working knowledge of the criminal justice system. 
• Experience providing case management to at-risk populations.  
• Experience utilizing a case management database. 
• Experience with emergency assistance programs.
 
ESSENTIAL FUNCTIONS
Case Management 
• Provide crisis intervention, information and referral resources.  
• Develop tailored case plans for services and periodic reassessment of the clients' situations and needs. 
• Complete all pertinent forms and progress notes for client's files, etc. and keep records current for monthly reporting. 
• Respect cultural diversity. Maintain confidentiality. 
• Ensure contact communication from criminal justice monitoring agency (i.e. probation/parole officers, etc.) to inform them of services PACE is providing and recommendations on any additional service needed.  
• Assist in training new PACE staff, interns and volunteers.
• Attend any and all meetings and trainings held in the community in reference to the reentry population.     

Income Support Coaching 
• Complete client assessment in initial interview and determine possible income supports available. 
• Identify, evaluate and assist CWF participant's eligibility to secure income and work supports including but not limited to Disability, TANF, SNAP, and Health Insurance. 
• Provide intensive coaching services to Center for Working Families (CWF) participants, assisting the client in developing individualized plans to ensure success. 
• Provide services (food, temporary housing, travel, clothing, etc.) which PACE can do directly or by referral to other agencies/individuals.
• Serve as the liaison to various types of emergency assistance (i.e. EAP, WAF, FEMA, CERF, ESG, and food pantries).   
• Provide consultation for other PACE staff working with clients around income supports, public assistance and housing.  
• Work closely with the CWF team of coaches, sharing and coordinating income and work support related information to the other coaches as they coach participants to economic self-sufficiency.
• Make follow-up phone calls after referrals are made.  Ensure all referrals are documented in case management system and in client's file.
• Remain knowledgeable about available income supports, barriers that affect those with felony convictions and ensure the staff are updated on any changes to the eligibility.  
 
Housing Coordination 
• Serve as the agency's lead housing coordinator by becoming knowledgeable about the housing landscape for those returning from incarceration.  
• Educate clients and staff on service availability, application procedures, client's/tenant's rights. 
• Develop partnerships with public and private services and providers networks, including relationships with landlords and apartment complex management. 
• Provide information on vacant housing, apartments and residential housing that fit the needs/reference of client. 
• Mediate and advocate for clients with agencies, landlords, apartment management entities, as necessary. 
• Attend CHIP meetings.

• These projects could change and evolve over time.  Additional projects may be added. 
 
QUALIFICATIONS
• Demonstrated ability and desire to provide confidential client care and guidance. 
• Patience, excellent verbal and written communication skills. 
• Strong organizational and interpersonal skills. 
• Demonstrated knowledge of Federal Fair Housing Laws.
• Ability to work independently and in team environment. 
• Ability to track and perform multiple tasks.
• Excellent time management skills.
• Punctual and dependable. 
• Flexible. 

REPORTING REQUIREMENTS:
• Provide monthly report of clients served, case load report and client success story to Director of Operations by the 5th of every month.  
• Provide follow-up on clients every 30 days.  Follow-up information is then noted in the client services database. 
• Document all client contact in ETO, including but not limited to employment placement, retention and termination and re-arrest.  
• Monitor that clients are enrolled in counseling, when appropriate, and are receiving bundled services.
 
PACE, INC. RESPONSIBILITIES:
• Always represent PACE in the most professional way.
• Attend and actively participate in all staff meetings.
• Submit requested information for Monthly Newsletter or other Development activity.
 
TRAINING:
• Attend relevant trainings when necessary.
• Obtain SOAR Certification 
• Remain educated about service agencies in Marion County.
• Attend Income Support Coach Roundtable meetings.
 
OTHER
• All other duties as assigned.
