The Rapid Re-Housing Case Manager will identify, evaluate, and assist veterans in the program to gain suitable, stable housing.  They will work in a team-oriented environment and collaborate towards a shared vision of success in improving the lives of veteran.

HVAF is working to end homelessness among veterans. Through a unique partnership with U.S. Department of Veteran's Affairs and many community organizations, we provide housing and re-integration services for homeless veterans and programs to prevent at-risk veterans from becoming homeless.

Duties and Responsibilities
a. Works with team members and other agencies to help identify, evaluate, and assist potential and current clients
b. Assists clients and their families with reaching housing stability
c. Conducts assessments and client intakes
d. Accurately completes and submits monthly and quarterly reports on time
e. Monitors client case management files and updates client progress notes within 48 hours in those files
f. Updates and maintains client information in the HMIS database
g. Locates appropriate resources for clients and makes appropriate

Education/Experience/Skills Required
a. Bachelor's degree in a related field; BSW or MSW preferred
b. Knowledgeable of the factors associated with homelessness, substance abuse, and mental illness
c. Knowledgeable of and able to provide referrals to other community resources
d. Demonstrated strong interpersonal skills
e. Demonstrated time management skills and ability to prioritize
f. Strong commitment to working on a team and towards shared mission and success
g. Ability to interact and communicate effectively
