SUMMARY:  Community Health Coordinator is an individual who will drive the overall outcomes of our health focus area.  The Community Health Coordinator will organize and engage our local senior population, focus on fresh vegetable and gardening efforts, and initiate family-oriented family health initiatives. The Community Health Coordinator is an essential human link and bridge between residents and seniors in the community and the Fay Biccard Glick Neighborhood Center (FBGNC) health initiatives. 

 

SUPERVISES:  Does not directly manage other FBGNC employees; however, will oversee the Food Pantry Coordinator and ensure all health principles are utilized within FBGNC Pantry.

                                    

OVERALL JOB DESCRIPTION:

         Manage day to day operation of FBGNC's Recycled Teenager senior program 

         Work directly with participants to recruit for What's the DEAL (family cooking) program 

         Coordinate participant needs with appropriate agencies and community support groups

         Oversea the food pantry and healthy eating components of the Family Campus model

         Improve the Community health (advocacy) of the community

 

SERVICE FUNCTIONS

•           Responsible for establishing trusting relationships with residents and their families while providing general support and encouragement for engagement

•           Providing ongoing case management process with the senior population

•           Developing unique activities that can be utilized in person or online

•           Work closely with apartment staff to help ensure that residents have comprehensive and coordinated information

 

 

ORGANIZATIONAL INVOLVEMENT

         Ensure FBGNC has adequate partnerships that include family programming, education, and possibly vocational programs 

         Ensure timely delivery and effectiveness of all services through community surveys, observation, and evaluation

 

OTHER DUTIES

         Performs any other related duties as assigned by the Executive Director or Director of Operations 

 

 

SKILLS AND SPECIFICATIONS

         Excellent oral and written communication skills 

         Highly efficient and comfortable using technology for programming and office support

         Ability to represent the agency to internal and external audiences through public speaking experiences

         Self-directed and motivated

         Excellent organizational and time management skills

         Ability to network, collaborate and foster partnerships with numerous organizations 

 

MINIMUM REQUIREMENTS:

         BA/BS/BSA  or some post-secondary education or training/experience specific to the targeted population

         One to three years' experience working with the target population in social service or related field preferred

         A degree in Public health, Social work, or Geriatrics is a plus

         Ability to pass fingerprint clearance test for all appropriate agencies (such as FBI, Department of Justice, and Child Abuse Index Services)

         The employee may be required to stand for periods of one hour or longer.

         Mobility required to travel up and downstairs and maneuver around other potential encumbrances to provide case management and home visits to program participants
